Person-Centered Planning provides a way of helping a person plan all aspects of their life. When entering our program, individuals and their authorized representatives meet with our qualified developmental disability professional (QDDP) to discuss the individual's current outcomes and personal interests. This establishes an understanding of how our service can further develop this individual and informs our QDDP on how to develop a plan of supports. Through this discussion, our QDDP can create an Individualized Service Plan (ISP) which clarifies the outcomes that Ruth's House shall work on with the individual and provides instructions on how to achieve these results. This plan is then disseminated to our team of Direct Support Professionals who will begin to provide our service based on these guidelines. Essentially, each individual's choices dictate the service that we provide, and the Individualized Service Plan supplies the framework from which we operate to deliver this service.
